9. Complete payment using an EastWest credit or debit card
10. Cardholder can use his/her card to avail of the offer for another person. 
11. Membership is non-transferable. Use of membership is not applicable to non-emergency cases such as patient transfers. 
12. Lifeline 16-911’s Membership Terms and conditions shall apply. Activation of membership application shall take 24 hours from time of registration.
13. The offer is non-transferable and is not convertible to cash or gift certificates.
14. By joining this promo, the Cardholder confirms that he/she has read, understood and agreed to the mechanics of the promo including, but not limited to, these terms and conditions. *Cardholders in good credit standing are those whose accounts are not delinquent, not under investigation due to suspected fraudulent activities, those whose EastWest credit cards are not reported lost or stolen and those who have not otherwise, violated any of the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards.
