1. The Supplementary Card Promo will run from June 1 to December 31, 2008.
   
   
2. This promo is open to all new and existing EastWest Bank MasterCard Principal Cardholders.
   
   
3. The principal cardholder is allowed up to a maximum of nine (9) supplementary cardholders under his account, including the existing supplementary cards issued.
   
   
4. All properly filled-out application forms must be forwarded to EastWest Bank Customer Service via fax no. 325-2377 or to any EastWest Bank branch near you. The form may be photocopied for a total of 9 applications. All supplementary card applicants must be at least 13-65 years old.
   
   
5. The primary cardholder will get one (1) Php 200 Gift Certificate of his choice (Jollibee, Anything Goes at Burgoo, Yellow Cab Pizza, KFC or Chowking) for every approved and activated supplementary card.
   
   
6. Please allow up to a maximum of ten (10) banking days for the delivery of the National Bookstore GC.
   
   
7. Approved Supplementary cardholders within the promo period cannot voluntarily cancel for one (1) year from membership date, unless earlier terminated by EastWest Bank. A pre-termination fee of P1,000.00 for every voluntary cancellation of supplementary card will be charged to the principal cardholder.
   
   
8. Annual Membership Fees for qualified supplementary cards are FREE for the first two (2) years only. In the event that the Supplementary account has been billed for the 2nd year, the principal cardholder should call 888-1700 for the annual fee reversal.
   
   
9. All transactions of the Principal and Supplementary Cardholders will be summarized in a single Monthly Statement of Account and will be sent to the Principal Cardholder's billing address. EastWest Bank shall have the absolute exclusive right to approve or reject credit card applications based on its internal policies or for reasons it deems justifiable. Thus, if the cardholder has no sufficient available credit limit EastWest Bank has the sole discretion to approve only a portion of the amount applied for.
